Abstract:

Abstract:

Background: District Management is emerging as a lynchpin for primary Healthcare system performance. However, delivery of district-level interventions at scale is challenging, and overlooks the potential role of Management at other subnational levels. From 2015-2019, Ethiopia’, s Primary Healthcare Transformation Initiative (PTI), aimed to build a culture of performance Management and accountability at the zonal level. This paper aims to evaluate the longitudinal change in Management practice and performance in the 19 zones participating in PTI, which included 315 districts and 1617 Health centers. Methods: Using data from PTI intervention (2018 to 2019), we employed quantitative measures of Management Capacity at Health center, district, and zonal levels, and quantified primary Healthcare service performance using a summary score based on antenatal care coverage, contraception use, skilled birth attendance, infant immunization, and availability of essential medications. We used multiple generalized linear regression models accounting for clustering of Health centers within zones to quantify (1) change in Management and performance during the two-year intervention, (2) associations between the changes in Management Capacity at the zonal, district, and Health facility level. Results: Adherence to Management standards at the zonal, district, and Health facility level improved significantly over two years (37%, P <. 001,18%, P <. 001,18%, P <. 001,respectively), as did the performance summary score (14%, P <. 001). Adherence at the zonal level in year one was associated with district level adherence in year one (P =. 04), and, over the two-year period (P =. 002), and district Management mediated the relationship between Management practice at zonal and Health center levels (P <. 001). Conclusion: Improvements in zonal-level Management practice were associated with significant improvements in district-level Management and performance in PTI sites. Investments in managerial practices at the zonal level may provide an immediate way to energize primary Healthcare system performance at scale in low-income country settings.

Abstract:

Background: Failed attempts to improve the delivery of Healthcare to communities show distinct flaws that have a higher impact during a major incident or disaster (MID). This study evaluates the concept of surge Capacity, which intends to achieve a balance between the needs and resources in affected areas by providing staff, stuff, structure, and system. Methods: A systematic literature review was performed according to the PRISMA statement and by using PubMed, Scopus, and Google Scholar, and related keywords. Results: There were limited publications about flexible surge Capacity (FSC). However, the sum of data obtained indicated the need for flexibility in expanding major incidents or disasters, demanding new resources, which may neither be available on time nor reachable due to infrastructural damage. Conclusion: FSC is a novel concept based on international guidelines. It refers to the extra and adjustable human and material resources that can be mobilized by activating nonprofessional but educated staff and different but accepted facilities in a fast, smooth, and productive way. Public Health and public education play an essential role in obtaining such flexibility.

Abstract:

The lack of Quranic approaches in the production of Quran-based humanities is still felt, despite the predominance of the philosophical approach in critical approaches to modern humanities, which has become very common in the Islamic world and in Iran for several decades. It seems that the Capacity of the method of thematic exegesis (al-Tafsīr al-mawdū'ī, Arabic: التفسیر الموضوعیّ) in this regard, can be used to bring the Holy Quran into the field of humanities. Thematic exegesis, according to whether its subject is inside or outside the Qur'an, has several functions in criticizing the structure of modern knowledge and scientific research, as well as the production of Islamic humanities. It is possible to make use of thematic exegesis of the inside of the Holy Book of Quran in order to “fundamental criticism” of the general fundamentals of humanities -the fundamentals of epistemology, axiology, ontology and anthropology- and it is also possible to use thematic exegesis for constructing and producing foundation of humanities obtained from the Holy Quran. It is used by thematic interpretation exegesis of the outside of the Holy Book of Quran -such as Shahīd (martyr) Sadr’s interrogational approach- to answer the problems of humanities and this process faces challenges such as the complexity and multiplicity of the “subject” of the humanities that exist in modern terminology of this field. It can be used in order to solve this issue in some cases to know the process of the change of the components of the modern of the conceptual history’s approach and also by analyzing the issue into several components can be shown that the components are not necessarily a product of the modern era and can be followed up in the Holy Quran and can be found up in the Holy Quran. The perspective of the Holy Quran, after that, regarding the modern subject in the desired field by the “combining” of the results obtained based on the thematic interpretation.

Abstract:

The current limited focus on Management in global Health activities is highly problematic given the amounts of financial and human resources that are pouring into Health system strengthening interventions and the complexity of clinical operations across settings. By ensuring that public Health and Healthcare practitioners in domestic and international settings receive Management training in their educational programs and that we build Management Capacity among individuals already in the Health workforce, we can begin to prepare for more effective Health systems strengthening efforts. Rigorous evaluation of Health systems strengthening and the impact of Management Capacity building is crucial to building evidence for the field.
